Fine Print
1. In-Shape Fitness and National Fitness may, at its sole discretion, offer the Perkville (the "Program") pursuant to which members may be awarded reward points through use of the Perkville website, and certain other activities that In-Shape Fitness designates from time to time.
2. Opting in to email communication from In-Shape Fitness and National Fitness allows Perkville to maintain the integrity of your rewards account. You may opt out of receiving email from In-Shape Fitness and National Fitness at any time you wish. But please remember, once you opt out, you will also not receive email updates on points.
3. In-Shape Fitness and National Fitness may, without notice, terminate the Program at any time, change the Program rules, benefits, conditions of participation, the goods or services offered in redemption of reward points, the number of points required to redeem goods and services, make no future awards, and void all prior points awarded, for any participant or all participants in the program, at its sole discretion, at any time. These changes may be made even though such changes may affect the value of the reward points already accumulated.
4. All reward points must be earned and used according to the terms and conditions of the Program. Reward points may only be exchanged for such goods and services from stores managed by In-Shape Fitness and National Fitness as determined by In-Shape Fitness and National Fitness at the time of redemption. Reward points may not be exchanged for cash. In-Shape Fitness and National Fitness assumes no liability for any prior offers which may have been presented, but which is not a current offer at the time of redemption.
5. In-Shape Fitness and National Fitness makes no warranties or representations, either expressed or implied, and expressly disclaims any and all liability (including consequential damages) with respect to the Program or any goods or services provided through the Program.
6. All interpretations of these rules, terms and conditions shall be at the sole discretion of In-Shape Fitness and National Fitness.
7. For In-Shape Fitness and National Fitness team members, gift cards and cash prizes may be subject to taxes at the federal and state level. Please consult with your tax advisor on taxability for income tax reporting.
8. In-Shape Fitness and National Fitness are not responsible for information submitted via a member for referral purposes. Dues credits are awarded only after a referral accepts the promotion provided by the referring member and stays a member for 90 consecutive days without membership termination or payment holds. Dues credits are subject to In-Shape Fitness and National Fitness discretion and are void if a referral cancels within 90 days of becoming a member. Referral accepts terms and conditions when submitting their personal information through In-Shape Fitness and National Fitness websites.
9. Must be a new member. Referral must be in our system before they join. Sub-members added to an existing membership do not qualify. Member must join through FIT CLUB Rewards referral link. Valid on month-to-month or 12-month term memberships. Referred member must be active 90 days and in good standing before reward is issued. Dues credit amount is equal to the dues for the main member only. Referring member must be an active member to receive the dues credit. Not redeemable for cash. In-Shape and National Fitness team members are not eligible for this reward. No bogus email addresses. Promotional offer subject to change. See website for complete details.
